I hereby authorize Reader Precision Solutions to deduct $10 from my paycheck each pay period as payment towards the outstanding balance for my apparel order, as indicated above.

I understand that these deductions will continue until the total cost of the apparel has been paid in full.

Please note that payroll deductions will commence following the arrival of the branded apparel.

**Note: Negative totals mean there will not be any payroll deductions.**

By submitting this form, you acknowledge and understand that the availability of items cannot be guaranteed. In the event an item you've selected is not in stock, the marketing team will contact you to look for an alternative option. If we do not receive a response within the specified timeframe in our communication, your order will proceed without the out-of-stock items or an alternative. Any credits assigned to unavailable items will be carried over to our next order. Additionally, please note that birthday and anniversary credits can only be used during the month of the credit. Once your birthday or anniversary month passes, the credit offered by Reader Precision Solutions will expire.
